In Canada, 20 children die and 1,800 are hospitalized from bicycling annually. Previous studies indicate that bicycle-specific infrastructure reduces injury risk to adults. It is important to determine whether these results extend to children given their physical and cognitive differences from adults. This study aims to investigate the associations of the built environment characteristics along bicycling routes to bicycling injury in children using a case-crossover approach. This pilot specifically seeks to examine and identify study design issues including: case-crossover methodology, implementation, interview process, and audit procedures in children presenting to the emergency department (ED) for future roll-out in multiple Canadian centres. Children (&lt;18 years old) presenting to the Alberta Children's Hospital ED with a bicycling injury, between July 11th and September 22nd 2017, were screened for eligibility. Eligible participants included those who lived and were injured on public property in Calgary, were riding a bicycle, and were not in a bicycling event nor trick or mountain riding. Eligible participants, or their parents\/guardians, were provided a Consent to Contact form, and following discharge, consenting patients were contacted and invited to participate. Participants were interviewed regarding their cycling route, including specifics of the injury site and two randomly selected control sites, one matched to intersection status of the injury site. These three sites were later audited by research staff blinded to injury location. Preliminary results indicate 66 (50%) of 132 patients approached with cycling injuries were eligible. Of those eligible, 49 (74%) provided a Consent to Contact form, 26 (39%) agreed to participate, and 23 (35%) were interviewed. Participants were male (70%), ranged between 3 and 16 years old (median: 10 years old), and travelled between 86 and 2740 meters (median: 479 meters) from trip start to injury site. 81% of participants were injured at non-intersections, while 59% were not involved in any type of collision. Pilot work indicated necessary adjustments of interview guide structure and question phrasing, site audit process, recruitment methods, and enrollment procedures. The case-crossover approach overcomes methodological limitations of previous bicycling injury research by controlling for variability in trip (e.g., weather, time of day) and participant characteristics. Consent to Contact forms permitted ongoing recruitment while maintaining patient privacy. Study materials require editing for young participants. Changes in protocol for the main study will include using online tools (i.e., Google Maps) for route measurement and documentation, participant recall and identification of control sites for auditors. Also, due to participants younger than 5 years of age having difficulty with interview procedures and recollection of injury, along with fewer opportunities for active transportation to school, this age group will be excluded. Current methodologies appear effective for collecting child bicycling route and injury data. Piloting procedures in younger populations will allow us to adjust the study protocol to increase logistical efficiency, acquisition of data, and the quality of the information collected.
